Where it is possible we will collect personal information directly from you. We need to collect personal information to provide the functions and services referred to above.
In the course of undertaking our functions and activities as stated above, it may be necessary (depending on the nature of the services we provide for you) to collect from and disclose to the following third parties your personal information (including sensitive information and health information):
a.your nominated treating doctor and any other treating practitioner who, by agreement is deemed necessary;
b.appointed return to work co-ordinator of your employer;
c.referring agency; and
d.persons/organisations engaged by Recovre.
Except as stated above or as otherwise required or authorised by law such as state workers compensation legislation, we will not use your personal information, collect your personal information from, or disclose your personal information to any third party, without your prior knowledge or consent.
If necessary personal information is not provided, we will be unable to do business with you.
If you believe that the privacy of your personal information at Recovre has been interfered with you may lodge a complaint by contacting us on 1300 550 276, EST 8am – 5pm, Mon-Fri or in writing to National Quality and Compliance Officer, PO Box 13345 Law Courts,277 William St, Melbourne VIC 8010. Your complaint will be managed and resolved through our internal Privacy Complaint Procedure.
If, however, you are not satisfied with the way Recovre has handled your complaint you may contact the Federal Privacy Commissioner.
