AUDIOLOGY ASSESSMENT FORM

An audiology assessment (hearing test) will be required for all NSW Police applicants.

This form outlines the hearing standards required by NSW Police. In the majority of cases this audiology assessment
will be completed at the same time as your medical.

You will need download this form and take it with you to the assessment only if you are having your hearing test completed separately to your medical. If you are not sure where you are having your audiology assessment done,
then please email policemedical@recovre.com.au.
